Welcome to our practice! Our new patients must complete paperwork, so we want to save you as much time as possible. You can print the below PDF documents, fill them out and bring them to your first appointment or you can type your responses within the document and submit the forms via email. Please send those to Info@herrickfamilydental.com and we will safely and securely download them to our office computer. We promise to keep this information confidential.
